Problem
A scaffold work area had unsuitable access and edge protection, creating a potential fall exposure for personnel entering the work front. A reported finding would continue to expose personnel until the field correction was complete.
My intervention
I raised the condition directly with contractor supervision during the walkdown and requested controlled access to the affected section until the scaffold team completed the correction.
Each observation was communicated as condition → risk → required action → responsible party, with agreed ownership and field follow-up.
Execution & verification
The scaffold arrangement was modified, inspected, and released before normal access resumed. I included the same condition in subsequent walkdowns to reduce recurrence.
Across the current assignment, approximately 18 findings covered scaffold and work-at-height conditions, access and housekeeping, hot-work readiness, barricading, lifting-area controls, cable and equipment arrangement, and PPE compliance. Findings were followed through daily coordination until correction.
Recorded closure
18 / 18 findings were followed through to closure. 14 were corrected in the same shift and the remaining 4 within 48 hours. No items in this set remained open at the reporting point.

Evidence scope
The figures refer to the 18 observations included in this current-assignment case. The scaffold example shows identification, escalation, contractor correction, inspection/release, and subsequent follow-up.
No internal HSE logs or site photographs are displayed. Previous-assignment safety performance is recorded under earlier field experience.
